Assessment fee: If an assessment is required to complete a form, an assessment fee will be charged in addition to the form fee.
Unlisted services: Unless a fee is stated above, all other uninsured services are billed at the physician hourly rate (minimum 5 minutes).
Forms & reports: Completion of any associated forms or reports, including follow-up, is charged at the physician's hourly rate unless otherwise stated.
Clinic Update: FHO+ and Clinic Changes Effective April 1, 2026
As of April 1st, there have been changes to how care is delivered in family medicine across Ontario. The Ministry of Health is introducing a new FHO+ model that focuses on direct patient encounters. As a result, you may notice some updates in how our clinic operates.
